Send your request and $8.00 to: Vital Records PO Box 5135 Tampa, Florida 33675 (telephone number: 813-307-8002) Include the person's: Name Date of death (and specify if you want it to be with or without cause of death) Your relationship to the decedent It should take around 10 working days for you to receive the document.
