There is a cite that will give you the address and office for any state you may be interested in obtaining info from. I have tried, but, was unsuccessful in getting a death certificate from Jacksonville for my grandmother. The address is: Office of Vital Statistics, Dept of Health, P.O. Box 210, 1217 Pearl Street, Jacksonville, FL 32231. They were very prompt in telling me they could not locate any information on my gmother--so I think you should hear from them rather quickly. I also should say that I believe my problem in locating info on my gmother is my problem, and not any problem with their system, etc. Also, I believe the cost was $5 for a death certificate. Their phone is: 904.359.6911--you should probably call for yourself for the charges. Also, I think if you follow links from the Ancestry.Com page, you can find the website that lists all the addresses and charges for the entire US.
