O. Box 210, Jacksonville, FL 32231-0042. They cost $5.00 (non-refundable) and it takes 3-4 weeks for processing. You can call 904-359-6900 (recorded msg) to obtain info. Basically it states: send a signed letter stating full name of deceased, sex, death date, city/county, your relationship, your address and your phone number with a check or money order for $5.00. If you can get the death cert, it should list the undertaker. You could then write to them and they may have more information in their files. You should also find the cemetery listed (if she was buried) and they may be able to provide more information. At the very bottom of the website at Step 7, you can click on "order form" under "Fax" and see a copy of the actual order form. The more complete the death date is, the better; however, the death day is not required if you have the month and year.
