Have you tried http://www.rootsweb.com/~cemetery/florida/duval.htm For a FL death certificate, see http://www.doh.state.fl.us/planning_eval/vital_statistics/deaths.htm Death certs are available from the Office of Vital Statistics, P. O. Box 210, Jacksonville, FL 32231-0042. They cost $5.00 (non-refundable) and it takes 3-4 weeks for processing. You can call 904-359-6900 (recorded msg) to obtain info. Basically it states: send a signed letter stating full name of deceased, sex, death date, city/county, your relationship, your address and your phone number with a check or money order for $5.00. If you can get the death cert, it should list the undertaker. You could then write to them and they may have more information in their files. You should also find the cemetery listed (if she was buried) and they may be able to provide more information. At the very bottom of the website at Step 7, you can click on "order form" under "Fax" and see a copy of the actual order form. The more complete the death date is, the better; however, the death day is not required if you have the month and year. I am not sure where your information comes from, but I don't think it is accurate for ordering directly from the Office of Vital Statistics. According to their website at http://www.doh.state.fl.us/planning_eval/vital_statistics/deaths.htm and their recorded message at 904-359-6900, you can order a photocopy certification of the original and the cost is $5.00 with additional copies costing $4 each. Additonal years can be searched for a fee of $2 per year with a *maximum* search fee of $55 reqardless of the total number of years searched. At the very bottom of the website at Step 7, you can click on "order form" under "Fax" and see a copy of the actual order form. I have not gotten one in the last 6 months, but at that time it was still as above. The copy was of the original and it cost me $5.00. Ann S. Hello, A while back I posted about an obit lookup. Several people told me that the library was going through renovation and would not reopen until mid-November. I don't know if the hurricanes have set this date any further back. What is the opening date for the library? Also, a couple of people emailed me off the list and said that they would do the obit look-up for me. I swear I saved the email. I can't find it, but I'll keep looking. I may have written the address on a post-it which fell off my computer screeen and is stuck to some paper here on my desk! So I'll go through all the paper here. Kathy, Were you looking for a list for the Yellow Fever Epidemic of 1857, 1877, or 1888? You may have had this in your email but I've lost it. I don't have a list but I have the book History of Jacksonville Florida and Vicinity by T. Frederick Davis, in which it talks about all three of the Epidemics. The 1857 mentions some names of the dead, the 1877 says very little at all, and the 1888 says quite a bit but only mentions about 9 or 10 names. Let me know if you'd like the information or the names and on which years. -Lauren.
