SmartMerge is a fully automatic merge (and doesn't ask for confirmation) so it will *only* merge the ones that FO can definately determine are duplicates. If you merge a person, but not the duplicate copies of the spouse, then the person will be linked to 2 copies of the same spouse. You have to merge each duplicated person in the database (this is why I advise people to *never* import a GEDCOM file into their database).
