Statistically, from 1911 to 2005, there were 1361 births recorded by the GRO where the surname of the mother and the mother's maiden surname are BUSH.  In the period 1911 to 1981 (which is as far as I have reached with my marriage transcriptions), there are just 138 instances of  a BUSH marrying a BUSH. So I am inclined to believe that the bulk of those 1361 births were illegitimate - in which case, the maiden name and surname will of course be the same.  I cannot readily think of any explanation other than the two you have already mentioned which would account for this.
