This was one of the Royal Navy's greatest blunders (Guinness Book of Naval Blunders 1993 ISBN 0-85112-713-4) which took place on 22 June 1893 when the Victoria was rammed and sunk by Camperdown. He formed the fleet into two columns 6 cables apart and then ordered them to turn inwards with fatal consequences. With such a well documented disaster who needs a death certificate?
