I am certain that it is 30 March 1855, which year also appears on the upper right typed and on the date registered. As to the location, it looks like Essington (that weird p looking letter is how a double s used to be written), which is a village in South Staffordshire and is 18 miles (15.4 km) from the village of Penkridge.
