I stumbled on this requirement over Christmas when I was looking for something on the Carmarthen Archives page: http://www.carmarthenshire.gov.uk/english/education/archives/pages/home.aspx "The [Carmarthenshire] archive service, together with the archive services of West Glamorgan, Swansea University, Pembrokeshire, and Powys are introducing a Reader's Ticket from the 1st January 2010." You can access the necessary forms via the above link. Seems to be Archives Network forms, so I would imagine the same forms would be in use via the above named archives.
