Completed forms should be mailed to: MONUMENT SERVICE Department of Veterans Affairs 810 Vermont Avenue, NW Washington, DC 20420 Be sure and fill in all the blanks, be especially careful in listing the company and regiment that your ancestor served in. They will verify his service. I failed to list the company mine served in but they telephoned me requesting this information. It only took 4 months to receive this headstone.
