Shirley H., to ease your mind, look for the bill they sent you for $138.00/  It has the phone number on there.  Call them monday, and make sure they got your form, and if you have any additional charges................I think they are satisfied with ours, as I haven't gotten any more bills in the last month or so, and Jim got refills on some of his meds................sam. --- They sent the form and I filled it out and mailed it some time ago..
