I have looked at a number of similar affidavits sworn in front of a Notary Public. They follow a formula <Signature 1> surr(ogate). The representative of the interested party for whom the affidavit is being signed. In this case the estate wishing to confirm the correct beneficiary. "In the presence of" Signature 2, Notary Public. There may be a seal attached or stamp duty. "In the presence of" is usually abbreviated to "pres" and "pot" should be confirmed against other affidavits of the same time and place. My guess is "presence of ..." The Notary Public. has to be present when the affidavit is signed for it to have legal effect.
