Thankfully you don't need much, but provide as much information as you can. I just got two dates of death by providing names and a range (1904-1906). Ct Vital Records was able to tell me where the deaths occured and the exact dates. As I mentioned in my first letter, checks MUST be made out to "Treasurer, State of Connecticut".
