    3. I would recommend checking with the CT Health Department CT Vital Records P.O. Box 340308 Hartford, CT 06134-0308 (860)509-7897 They have a computerized index to all deaths from 1897 - present. They can handle the request by mail and if you mail them a check for $5.00 made out to "Treasurer, State of Connecticut" they can issue you a copy of the death certificate.
