If a standard form were decided on, it would not be hard to incorporate new information with a minimum of typing. Material could be simply placed in a database which is searchable (I don't know how to do that) or into a table in html form- which most wordprocessors can do. some suggested fields: First name Last name (for spouses) Location State date type of record information contact/submitter name
