Here were my thoughts on the OHIO thing. She is rumored to be a descendent of The Daniel Boone. He was a Quaker from Pennsylvania. Some of the first settlements in Kentucky were named "Ohio" and then later Ohio, County. So, I was assuming that it may be a mistake. Her daughter (Laura) also, lists her mother's birthplace as PA in the 1900 census. Just a long shot, but I cannot find her in anything other then the 1880 census and have no clue on her parentage.
