Hewlett-Packard used to make a device they called "CapShare," a hand-held scanner that you would scan down the left side of a page then in a sliding and sweeping motion up the right side (the scan window is 4.5" wide). Software in the unit "glues" the halves together in a single text or PDF file (doesn't matter what the text or image is, it gets glued together with no discernible "line down the middle"). I have one of these things and, I'll tell ya, I wouldn't give it up for anything -- works great. They're supposed to be releasing a new model, and, based on my experience with the older one, I'd buy the new one in a New York Minute. Ain't cheap, but sure makes life easy at the library...
