Five Holy Martyrs (Catholic Church) at 43rd and Richmond St was founded in Nov 1908 (Polish ethnicity). In 1908 parishioners worshiped at Davis School at 3014 W 39th Place and in Dec 1909, the church was relocated to 3141 W 41st Street. Records are at Five Holy Martyrs, 2905 W Pope John Paul II Drive. Doesn't look like any of the addresses fit plus the date of church establishment. No other Catholic churches are located in the general area of interest in 1905 according to "Locations of Chicago Roman Catholic Churches" by Jack Bochar. Regards, Jim Sladek
