Cook was born in Cumberland Gap, Kentucky on March 28, 1821 and died in Richwoods, Washington County, Missouri on August 19, 1908. He married first Susan Isabella Kincaid on June 21, 1846 in Old Mines, Missouri. He married second, Marie Margaret Boyer on April 15, 1861 in Washington County, Missouri - St. Joachim Church.
