Here is some better information on birth and death certificates. Birth certificates for anywhere in Colorado from 1957-present are available, and all Mesa County records. The cost is $15, plus $6 each for additional certified copies of the same record, payable by cash, check or money order. You need full name, father's name, mother's maiden name, date of birth, place of birth, your relationship, and the reason for your request. You also need ID according to the recording. For death certificate, which is the same cost, you need full name, date of death, date of birth, father's name, mother's maiden name, and relationship. I imagine if you don't know all the information, they would probably still be able to retrieve the records.
