Madam: Receipt is acknowledged of your Eastern Cherokee Application No. 39579, which is found to be incomplete. As the application you have forwarded is written with a lead pencil and this Office prefers that the same be written in ink, there is enclosed herewith a blank which you are requested to fill out with ink and return to this office without delay, observing the following instructions: (1) In answer to question No. 8, give the name and age of your husband. (The original asked "Name of wife or husband" and she put her and her husand's name) (2) In answer to question No. 9, state if he belongs to any tribe of indians, and if so, to what tribe. (She entered Cherokee) (3) In answer to question 11, give the full name of your father and of your mother. (She gave father: Cloud, Mother; Riley) (4) Sign and swear to this application before a Notary Public or other proper officer, sicning your own full name not initials. (5) Have the supplementary affidavit signed and sworn to by two witnesses. The other blanks you will be able to fill out without any instructions.
