    1. Generic Document Date - which?

    3. When an obituary/death notice is published in the paper, there are three or more distinct dates - the date of the death, the date the notice was published, the date that the notice (document - see Liz I'm getting there) is discovered. Additionally in some cases, there are more dates - when the relative told of the death and the date that the notice was entered into Clooz. Which is the best date to use?

You would only have two dates from the document, most likely. The date on the first form, the generic document form, should be the date of publication or the date something was recorded, because that refers to the document itself, not what's happening in that document. The death date would be recorded on the detail screen for the person. The other dates are not relevant to the document. The date the notice is entered into Clooz will be recorded by Clooz, so you don't need to worry about that one.
