It doesn't matter what the last name on the form is. However, it need to be the one taking the DNA test and that has to be all male from the Boyd line you are trying to connect too. I was told that you would receive a DNA kit and you would just swab the inside of your mouth, then send it to a certain lab and they do the testing. Ruby
