It appears this is actually a list of everyone who was ever assigned, rather than that of the original crew. Also, John Worden (who was in command) and William Jeffers were lieutenants at the time. There's at least a couple missing from this list: David Cuddeback (an original crewman) is listed in the Navy OR as a survivor, and the bio on Siah Carter (see next link) says he was, too.
