The Monument Service, National Cemetery System, will furnish free of cost a headstone or marker to be placed at the unmarked grave of a member of the Armed Forces of the United States or the Confederate States of America who either died in the service or who was discharged or released therefrom under conditions other than dishonorable. 4-2 M21-1, Part VII July 24, 1990 b. Application for graves of veterans in private cemeteries will be made on VA Form 40-1330, Application for Standard Government Headstone or Marker, which will be submitted in accordance with the instructions printed thereon. The nearest relative, or funeral director, or other person (barring intent of the relative to erect a private monument) may execute the application. No application is required for burials in national cemeteries; headstones or markers are supplied automatically upon submission of a VA Form 40-4956, Record of Interment, by the national cemetery Director. c. The Government will prepay shipping charges to the extent indicated on VA Form 40-1330. Costs incident to transporting the stone to the private cemetery from the consignee address, and to erection at the grave are not payable by the Federal Government. At Government cemeteries, all costs are borne by the Federal Government.
