My enrollment packet contained interviews with husband(Choctaw) and wife(white), two birth records of their children, proof if their marriage, two testimonies from people proving that the marriage was legal, letters sent from the BIA to the husband and wife, and a sheet stating that the husband's mother was Choctaw (his father was white). The interviews were very interesting they stated previous marriages and children born of them, the years of their births, parents, etc. It was a total of 27 pages and it cost me $13.50.
