I considered this to be a minor issue, and have not done a bug report. Ira Believe I found the reason, the cause was embedded pictures some which were jpg files with rather heavy volume. You should make an recommendation as to what max size to use. So despite me having 32mb ram and plenty of free space, this caused the program to generate very slowly and skip generating the first page, when I removed that particular picture, it generated ok, so now I take alook into picture size in general to reduce size.
