    3. First question, do you need a certified copy, or is an informational copy sufficient? Same price, but if you need a certified copy you will need to go thru many more hoops. The form necessary can be found at http://www.dhs.ca.gov/publications/forms/pdf/vs112.pdf I would not request the death certificate from the state, the county is faster San Mateo County Recorder 555 County Center, 1st Floor Redwood City, CA 94063-1665 http://www.co.sanmateo.ca.us/smc/frameset/0,,14094288_14099386,00.html?fpURL =http://www.smcare.org/records/birth.asp#top (a two line address states) Death Certificates Purchase a certified copy of a birth or death certificate in person at the County Clerk-Recorder's office at 555 County Center, First Floor, Redwood City, CA 94063-1665. Payment types accepted in the office are cash, checks, and credit or debit cards (Visa and Mastercard only). Application Mailed requests for certified copies of birth and death certificates must be accompanied by a notarized application. (Application form will require the Adobe Acrobat Reader which may be downloaded for free here.) Mail the application with the required payment in a check or money order only (payable against a USA bank) to the above address - Attn Vital Records. Make checks payable to San Mateo County Clerk-Recorder. Expect the certified copy by return mail in five to 10 working days, 7 to 14 working days if outisde the U.S. Certificates from the years 1865 to 1965 may require an additional one to two days for processing. The certificate may be shipped overnight at the applicant's expense. Please include a prepaid return express envelope or airbill with your request.
