Both the Access to Information Request Form and the Personal Information Request Form can be found at the following website address: http://www.tbs-sct.gc.ca/gos-sog/forms/Acc-pri/Request-Frms_e.html It may be useful to consider that the submission of a request for access to personal information (such as that contained in the census documents) is a formal process that can, should access be denied, be appealed to the Office of the Privacy Commissioner.
