I tried to get the ATI form from the URL Gordon gave in his recent e-mail to the list. Unfortunately it printed out all squished with no space to "Provide Details" etc. In fact the form looks different than the one I filed last year. I suggest you, and others, follow Gordon's suggestion and use http://www.tbs-sct.gc.ca/gos-sog/forms/Acc-pri/350-57_e.rtf This form is adapted for those who do not have Adobe Acrobat. I printed the form -- there is AMPLE space to type or write the suggested paragraph. One is able to type it when on the screen, then print TWO COPIES -- one to mail, one to save. Should YOU run out of room, put an ARROW pointing to the right hand side of the page, with (see over) Finish your statement on the other side -- NEVER use two pages, one page is likely to be dumped.
