The form may be downloaded from http://www.tbs-sct.gc.ca/gos-sog/forms/Acc-pri/350-57_e.rtf In filling out the form 'Federal Government Institution' --- 'Statistics Canada' 'Provide details regarding the information being sought'. --- My wording was "As a family historian and genealogist I have a requirement to view and obtain information regarding my ancestors from Historic Census Records. The Privacy Act of Canada, and Regulations attached thereto, provide that information from Census may be made available to any person or body, for purposes of research, 92 years after collection. This message is to be considered my Request, under the Access to Information Act, for access to schedules of the 1911 National Census of Canada. Thank you" Feel free to use your own wording. I would, however, keep the request in general terms seeking access to the 1911 schedules of Census and also avoid making a request for specific individuals. If you seek information on specific individuals you will likely be informed to submit a request for a 'Census Pensions Search' at a cost of $48.15 per individual. 'Method of access preferred' --- No need to respond to either option. We expect the request to be refused so neither response would be applicable. 'Name of applicant' and address information is self-explanatory. Mark the box indicating the request is being made by 'a Canadian citizen, permanent resident or another individual present in Canada,'. Sign and date your request. Keep a copy of it. Mail your request, including a check or money order for $5.00 made out to the 'Receiver General of Canada', to: Pamela White, ATIP Coordinator Statistics Canada R.H.
