Thus, those records of birth, marriage and death are available. Here's the information I have on that issue: New Brunswick, Canada The Vital Statistics Office registers births, marriages, deaths and stillbirths in New Brunswick. What certificates are available? Birth and marriage certificates are available in a wallet size or in a long form certified copy. Death certificates are available in a certified copy only. What information is recorded on each type of certificate? The wallet size copy of a birth certificate includes;  name of individual  date of birth  place of birth  sex  registration date  registration number, and  date issued. The long form certified copy includes;  name of individual  date of birth  place of birth  sex  registration date  registration number  date issued, plus  the names, and the province or country of birth, of the parents. What information is included on a marriage certificate? The wallet size copy of the marriage certificate includes;  name of the bride at the time of marriage  name of the bridegroom at the time of marriage  date of marriage  place of marriage  registration number  registration date, and  date issued. The long form certified copy includes;  name of the bride at the time of marriage  name of the bridegroom at the time of marriage  date of birth  place of birth  sex  registration date  registration number  date issued, plus  the province or country of birth of the bride and bridegroom. A photographic print of a marriage registration is available with the written consent of one of the parties to the marriage. What information is included on a death certificate? The long form certified copy of a death certificate includes:  name of the deceased  date of death  place of death  date of birth  province or country of birth of the deceased  registration number  registration date, and  date issued. What information is required to obtain certificates?  Birth Certificate:  Name of individual (name registered at birth)  Date of Birth  Place of Birth  Father's name  Mother's full maiden name  Marriage Certificate:  Name of bride at time of marriage  Name of bridegroom at time of marriage  Date of marriage  Place of marriage  Death Certificate:  Name of deceased  Date of death  Place of death  If known, parents' names, spouse's name, and/or date of birth of deceased What are the fees for obtaining a certificate?  Wallet size copies - $20.00  Certified copies - $25.00  Photographic print of a marriage registration - $25.00 How to apply for a certificate You can obtain a certificate by providing:  the required information;  reason for request;  relationship to the person named on the certificate;  type of certificate requested;  a cheque or money order made payable to the Minister of Finance in the appropriate amount or by providing your VISA or MASTERCARD number with expiration date;  full return mailing address and daytime telephone number.
