Carolynn, Where did you write for this death certificate? I know I would send another letter with the information you requested and the incorrect information you received. I had something similar happen in Washington State. I think my mistake was requesting two different certificates on one letter. I did write again and gave my phone #. I received a phone call and the correct death certificate. I don't know what it's like in Canada. Maybe someone on the list can help with this. At $25 a shot I'd at least like the wrong one in the right year. I did finally get one for my grandmother though with some info I needed. Out of $75 I got one..
