No, that wouldn't be the right form. W.C.B. is a provincial program for starters. What you need is an authorization form from your clients, authorizing you to act on their behalf. (And you could ask your MLA's constituency assistant for a copy of their standard authorization form and make up a similar one) To get information from either level of government you are still going to need your client's signature requesting the information. With an authorization form though you can get information over the phone or have the requested information sent to you. With W.C.B. the best people to contact are the W.C.B. Ombudsman's office at 604-276-3053. Peter Hopkins is the Ombudsman and for cases that fall within his mandate he has a very quick turnaround time. His secretary (Vi, as I remember) is also an excellent and helpful person. There are about five others working there and all are good. You do know that if this is a long distance number you can call through Enquiry BC and be connected free. The number for the Lower Mainland is 604-660-2421. For elsewhere look in the blue pages in your telephone book. They will connect you for free with ANY provincial government number in the province, including colleges, etc. Working on W.C.B. issues is very tough work. Good luck!
