Hello List, Does anyone have a link to the form needed to apply for a delayed birth certificate? My dad was born 4 June 1912 at home in Parlier, Calif, and we have never been able to find a birth certificate for him. He is still living but I wanted to file for a delayed birth cert. before he passes on. Any info greatly appreciated.
