Received 2 copies of mother-in-law's death certificate today. She died on Oct 10th. It had 3 errors in it. The mortuary [Chapel of the Chimes in Hayward, CA] said that they would have to ammend the death certificate. I sent the incorrect death certificates back to the mortuary, requesting the ammended ones. Am I to understand that when one requests an ammended death certificate that what we receive is a 2-page noterized certificate [the incorrect original and a seperate page with the ammendments/corrections on it]?. The mortuary had filed the death certificate the day before I was able to send them the correct info on my mother-in-law. I want to be sure Alameda County records are correct, which is why I am following up on this. I mearly wishing to make sure that the info on the death certificate is accurate [it is a legal document] so that others requesting it have the corrected information, not erroneous info.
