In my one & only foray into obtaining a death certificate in Ontario, I recall the following. Cost- approx $25.00 Information- date of death,-place of death, if female, maiden name- father's name. There are 2 forms of certificates. The "short" one is easily obtained. The "long" form, which provides details of the cause of death can only be obtained if permission is given by the closest living next-of-kin. In my case, I was it so I just certified that as fact.Took about 3 weeks to arrive. All details should be on the website.
