A couple years ago a wonderful lady at the Kitchener FHC was going to Ottawa and got me a death certificate (unfortunately the wrong one). I would like to get a copy of the right one and a brother's also. Can you tell me what is the correct way to go about this? Hello If you want a copy of a BMD registration document, write to the : Office of the Registrar General, P.O. Box 4600, 189 red River Road, Thunder Bay, Ontario P7B 6LB. Phone # 416 325 1838 They charge $15.00 CDN for a (2.5 x 3.75) certificate. A certified photostatic copy of a registration (8 x 14) is $22.00 CDN. A five year search (letter format) is $15.00 . Phone or mail them to get an application form and go from there. The above information I copied from a 1994 fee schedule. I phoned them to try and get the latest fee schedule but all I got was an answering M/C menu ( you can phone and leave your address and they will mail you an application form). I was thinking if you want to save a long distance phone call, send me you mailing address . I will phone them from here and leave your address. The Ontario Archives in Toronto has copies of the BMDs on film but they do not do research or copying. Meaning you would haveto hire someone.
