I tried exiting BK again and going back in with same problem.  But, when I tried printing a group sheet for a "family of" rather than the range of number and the family of printed.  Then, I tried the range of numbers report again and it worked fine, index and all.  So, I've got what I needed, but not sure why I had to do what I did to make it work.
