I must have copied it, was a long time ago. Haven't found my copy yet. Still not all unpacked. I am trying to remeber what the pages look like, does it look like a small book page, if so I copied it from the book. As far as I remember no one sent it to me, it was in the early nineties I know it was before 1995 I found the information. Will try and find the rest of my papers. They are working me more hours then I expected to so will get back with you as soon as I can.
