ON THIS thirtieth day of December, A.D. one thousand nine hundred and sixteen, personally appeared before me, a NOTARY PUBLIC, within and for the County and State aforesaid, duly authorized to administer oaths, Eugene B. Pinney, aged 38 years, a resident of No. R.F.D. 3 Street, Chester, County of WINDSOR, State of VERMONT, and Bertha M. Pinney, aged 30 years, a resident of No. R.F.D. 3 Street, Chester, County of WINDSOR, State of VERMONT, well known by me to be reputable and entitled to credit, and who, being duly sworn according to law, declare in relation to the aforesaid case as follows: We have lived in the same house with Davidson M. Barr for the past five years, and have nursed and cared for him that length of time, more or less. For the past year or more he has required the assistance of another person. He has not been able since then to dress or undress himself and also requires help in getting into bed and getting up. It would be unsafe for him to be left alone in the house, for any length of time, lest he should attempt to walk across the room and fall on to the floor if such should occur he would not be able to get up again, and he has to be watched while we are in the house with him.
