Ditto here on the disappointment with the form. It took all of about 2 minutes to fill mine out and get it back to the mailbox. No info requested there that will tell anyone in the future anything important about you, unless they consider it important how many people were living in your household. The names, ages, etc, are already available so many places. Sure glad those folks in 1850 asked more pertinent questions!! Sherry
