Hi Brigitte, I couldn't find a death certificate on Ancestry. Phillip Otto is buried in Crown Point Cemetery in Indianapolis and they did have this information on their web site (see attached Genealogy Form) about how to obtain a death certificate. They are also available from the local health department in the county where the death occurred. Proper identification is required. Cost: Death certificates are $8.00 each. Additional copies of the same certificate are $4.00 each. Written requests can be paid by personal check or money order.
