I'm not sure that he ever resided in the Phoenix or Maricopa county area. I know that at one time, they all lived in Bryan, Ohio (Williams County) and they relocated to the west in the 40s.
I recall a story that ran in the Arizona Republic, maybe 1-1/2 years or so ago, about the restoration of a cemetery at the Arizona State Hospital. The story emphasized a Congressional Medal of Honor winner who was buried there. Maybe somebody in facilities management at the hospital can provide you with the names of people buried there. It is also about 1 1/2 miles due south of Mint Wash. Because of the terrain, many areas that surround Prescott have their heritage steeped in ore mining. My collection is entirely devoted to city and telephone directories from 1931 through 2002. I noticed that you spelled Grady's last name "WAGGONER". I presume that is in error and was intended to be "WAGGENER"? I continued my search of my collection and found no WAGGENER listings for Phoenix 1955, 1957, 1959 and 1960. I also found no WAGGENER listing for 1966 Glendale metro which includes most suburbs west and northwest of Phoenix. I found no WAGGENER listings in my Tempe/Mesa/Chandler/Gilbert directories for 1960, 1961 and 1964. I did find one WAGGENER listing in my 1968 Scottsdale directory; a "John D and Marjorie M WAGGENER" and their 3 children residing at 6616 Cardinal Drive, Paradise Valley. He is listed as a Pathologist at St. Joseph's Hospital. In my 1975 Phoenix metro phone book, there are three WAGGENER listings, one a Gerald R. of Scottsdale, two listings for Dr. John D. (person noted in previous paragraph, address for both business and home - home address unchanged) and a W. C. WAGGENER of Mesa. This concludes the breadth of my resources and I'm sorry I wasn't able to find more for you. You might consult the National Archives for the Selective Service Records of the World War I era. Voter registration records are public record. That's one of the reasons they are often at the center of election controversies; i.e. deceased people "voting" and election opponents publicly proving something was "fishy" with a vote count because a registration record can be checked to death records. I would start with either the Maricopa County Recorder (Clerk) or the Office of Registrar of Voter's. Each government entity refers to it's registrar differently. The Recorders office is located at 111 South 3rd, Phoenix, phone 602-506-3535. The Registrar of Voter's is also located at the same address, phone 602-506-1511. Each state's policy on archival information on deceased persons is different and in a constant state of change; generally becoming more restrictive and difficult to obtain as the public's appetite for privacy increases. When contacting any government agency I would let them know you are a descendant and that you can prove it should they so require. I have no information other than the knowledge that Eleanor Elena Waggener survived her husband, Walter P. Waggener, who died in 1938 in Albuquerque, Bernalillo County, NM. I also have information that her son, Walter Grady Waggener, married Vera Younger 04 Jun 1910 in Texas and that the family lived in the Phoenix area some years later, though they divorced. There is the possibility that Lena moved to the area where her son was living. The name is Waggener, not Waggoner.

I searched several different ways and found nothing. Therefore, I would suspect your brother died elsewhere. According to the SSDI on-line records your brothers date of death was confirmed to SSI with the presentation of a death certificate by some party. Perhaps SSI kept a copy of the death certificate. They also show his last known residence was a code "(72)", whatever that means. I would suggest you invest $27 and write SSI for a copy of his SS file and contents. It's a little pricey, but the file may include all the information you need to pursue your research more quickly. There is no listing for a "Bertha OUTLAND". As there are only 3 OUTLAND surname listings in the directory and this one is the only "Keith", it seems reasonable this may be your research subject. Using the residental address you could check with the County Clerk to see if he owned the property and then trace the property forward to his date of death to see who the property transferred to. Additionally, most property owners are registered voters, and if so for "Keith", then possible genealogical information may be found in his voter registration record.
