    3. Hi Cheryl I believe the original birth certificate will remain as it is. The name change takes effect only from the date of change. This is so in my husband's case - in England - anyway. Regards Sandra

    3. My grandfather's birth certificate is in his original name with a notation that name was changed by deed poll and the new surname at the bottom of the certificate. He was born in NSW 1899 and changed his name sometime before 1930.
