Hi Maria, I have a death certificate from 1955 and the following fields are available for information to be inserted: Name of Deceased Occupation of Deceased Last Residential Address of Deceased Sex Age Cause of Death Name of Medical Practitioner Date MP last saw deceased Place of Birth Number of years lived in WA Name of Father Father's usual occupation Conjugal condition of Deceased Place of Marriage Date of Marriage Name of Person to whom married Christian (or equivilant) Name and age of children of deceased, in order of birth Date of Burial or cremation Place of Burial or cremation Name of person certifying to disposal of body Name of certifying crematorium official Name of Officiating Minister Religious denomination of Officiating Minister Name of Witnesses of Burial or Desposing of Body at Crematorium Informant Description of Informant Address of Informant Name (or Signature) of Informant Registrar's date and place of Registration.
