Thanks Di for your continued interest. Julie Hilton did get the certificate for a Justine Muller who died age 95, supposedly dying on the date in 1915. Her maiden name was different, as were her children's names. I also sent for the certificate, but the lady from the Registrar wrote to me telling me it was not consistent with my details sent to her. She was most helpful but I did not ask for all the details you are asking. I have emailed Julie asking for details on her printed copy. I have received nothing from the Registrar from my application, so may phone and ask my money be used to purchase the marriage certificate. We do not know if it was a hospital death, who was the informant etc, but maybe Julie will be able to help there also. I am not sure about the electoral rolls either.
