Hi John and list It is my understanding that special application can be made to the NSW Department of Health for the health records of a deceased relative. I don't know what dates are involved. I was told in 2004 by someone on this list that you need:- *a certified copy of the death certificate *certified copies of birth, death and marriage certificates to prove your descendency *the full name of the hospital where treated.
