My grandpa's name was Howard Clive Engstrom. He obtained a copy of his birth certificate and the name was written out as Howard Olive. I've seen the original on microfiche and the name is clearly Clive. Someone who was not really concentrating on the job had misread the the original handwriting when it was transcribed onto the certified copy.
