His Social Security Card was issued in Iowa and the number was 481-05-4129. The death index does not list his residence at time of death. I have a form that you can sign and send to the Social Security Administration and they will send you his records that will tell you where he died and where he was buried and much more information that will be very valuable to you. The cost is $27.00 and is well worth the money. When you know the city where he died you can obtain an obituary from the local paper. If you would like me to send uou this form, just let me know.
